Case 1 - First time application
- Apply for the card : ATM card - application form
- Card PIN will be issued automatically to corresponding address;
- Card needs to be collected in person;
- Authorize another person to collect card : ATM card collection authorization form
- Please send signed forms to your banker for processing;
- Please send signed authorization letter to authorized person;
- Apply for the card : ATM card - application form
- Please mark "non-dilvery of card" in Section A - Others;
- Card PIN will be issued automatically to corresponding address;
- Card needs to be collected in person;
- Authorize another person to collect card : ATM card collection authorization form
- Please send signed forms to your banker for processing;
- Please send signed authorization letter to authorized person;
- Apply for lost card PIN : ATM card - PIN enquiry
- Card PIN will be issued automatically to corresponding address;
- Please send signed forms to your banker for processing;
- Client can authorize us to collect the card.
Please just sign authorization form and send to us by mail or courier. - Usually, the bank will keep the card for around 1 month.
The card will be cancelled if not collected. Client may need to apply again when he is available to pick up.
